Help! It doesnt work on new phone!

I used to love this app on my old iPhone. I recently upgraded to the iPhone 6s+ and when I go to put in the information for my child I can only put in the name. The other fields wont let me input anything. Has anyone come across this or know how to fix it? If really like to use the app again.
